    Dominique N. Howard, FNP-C is a native of New Orleans, LA with a knack for eliminating barriers from accessing healthcare for vulnerable populations. She has a diverse clinical background, with over 17 years of nursing experience in Oncology, Wound Care, Medical -Surgical care, Interventional Radiology and Trauma. She has thrived in the primary care setting providing care to adult, geriatric, outpatient and inpatient populations. Howard Adult Health is expanding in order to meet the needs of our communities, you may be scheduled with another provider occasionally.

  • How Do I Become a Patient & Schedule an Appointment?
    For New Patients: New referrals should go to our intake email or fax: info@howardadulthealth.com Fax: 680-778-6034 After intake documents are received and insurance verified, you will automatically be placed on the schedule to be seen within the next week. New Residents will be seen within a week of move in To note: For immediate move in's requiring PMEs, chest X-rays or medication reconciliation please call the office to schedule as soon as possible at: (877)-417-7873
  • I'm already a Patient. How often Will I be Seen?
    For Established Patients: We will attempt to schedule your next follow-up appointment prior to ending an appointment. Typically, we will follow up every 4-6 weeks based on your needs, unless needed sooner.
  • What's the Difference Between This & Home Health?
    Home Health is usually catered to short term care related to a specific amount of time when one is discharged from the hospital. In addition to this, you need a prescription to be seen by Home Health. Howard Adult Health is a more long term solution and geared towards keeping you out of the hopsital.
